Age-Period-Cohort-Income analysis of the number of chronic diseases morbidity to Korea in 2010-2018

Abstract
Objectives: This study aims to evaluate the health inequality of multiple chronic diseases according to household income, age, period, and cohort in Korean adults. In addition, through this, it is intended to provide basic data for preparing policies to reduce the burden of chronic diseases on individuals and nation as a whole.

Methods: In this study, Age-Period-Cohort-Income(APCI) analysis was conducted in consideration of age, period, cohort, as well as household income, to identify health inequalities using data from the 5th to 7th Korea National Health and Nutrition Examination Survey. Of the 72,751 subjects, a total of 53,192 subjects, excluding minors and missing data of 19,559, were selected for the final analysis.

Results: The age effect showed a trend of increasing with age, and the period effect also showed a trend of increasing with time. On the other hand, the cohort effect showed a decreasing trend toward the recent cohort compared to the past cohort, and the income effect also showed a decreasing trend toward higher income compared to low income.

Conclusions: As a result of APCI analysis, it was found that there was a significant difference in the trend of each effect on the prevalence of chronic diseases. This study is the first study that considers household income with the existing age, period, birth cohort. It helps to understand multimorbidity and the average number of chronic diseases and enhance the need to establish policies to resolve health inequalities.
